Job Description

EA Family Services is seeking a compassionate, motivated, and dedicated Case Manager to join our Transitional Housing Placement Plus Non-Minor Dependents (THP+NMD) program serving young adults ages 18-21 in the Yuba City area. This is an exciting opportunity to support transition-age youth as they develop the skills, confidence, and independence needed to build successful futures. About the Role As a Case Manager, you will provide guidance, advocacy, and support to young adults who are transitioning from foster care into independent living. Working as part of a collaborative team, you will help participants achieve personal goals, access community resources, and develop the life skills necessary for long-term stability and success. Qualifications Required Bachelor's degree in Social Work or a related field Valid driver's license Ability to pass DOJ, FBI, and CAIC background clearances Strong written and verbal communication skills Ability to work effectively within a team environment Preferred Two years of case management or related experience Experience working with transition-age youth, foster youth, or vulnerable populations What We're Looking For The ideal candidate is: Passionate about empowering young adults Organized, dependable, and self-motivated Skilled at building positive relationships Professional, empathetic, and supportive Committed to helping youth achieve independence and self-sufficiency
Compensation & Benefits Pay:
