Description:
United Services is seeking a compassionate and dedicated supportive Case Manager to provide assessment, advocacy, and case management services to individuals participating in our supportive housing program. This program serves individuals who have experienced homelessness and are living with disabilities, including mental health disorders or substance use disorders. provides person-centered services that promote housing stability, self-sufficiency, and overall well-being. The supportive housing programs follow the Housing First model and provide person-centered case management services that meet individuals where they are, helping them build stability, strengthen independence, and achieve their personal goals.Location:

Our supportive housing program focuses stable housing while providing individualized support tailored to each person's unique needs and goals. We work from a client centered, collaborative approach that empowers individuals to meet their goals. What You Will Do (Day-to-Day): Provide assessments for assigned clients. Develop, monitor, and update Community Service Plans in partnership with clients. Build and maintain positive, professional relationships with clients, referral sources, community partners, and service providers. Coordinate and facilitate access to community resources and support services. Initiate and monitor client participation in agreed-upon services to ensure continuity and appropriateness of care. Provide individual and group counseling, education, support, transportation, and daily living skills development services. Advocate for clients and support continuity of care across providers and service systems. Assist clients in maintaining housing stability and achieving greater independence. Maintain accurate, timely, and complete records, documentation, and reports. Conduct daily interactions and services with professionalism, respect, and a person-centered approach.Schedule:
Full-time, Monday through Friday, 8:00 a.m. through 4:30 p.m.Requirements:
Bachelor's Degree or Associates Degree with 2 years of relevant experience required.Hourly Pay Range:
$19.00- $ 21.50 per hour (based on experience) Bilingual
