The Youth Diversion Team Case Manager provides assessment of issues, referrals/follow-up, scheduling, panel recruitment and linkages with appropriate community resources in order to support the Youth Diversion Team and its goals.
Qualifications :
- Bachelor's Degree in Human Services, Criminal Justice, Social Work or a related field.
- Effective organizational, written and verbal communication skills
- Experience working with children and/or families and group facilitation skills a plus
- Strong counseling, crisis intervention and problem solving skills
- Respect for and understanding of diversity issues
- Computer experience
- Must have and maintain a clean CT Driver's License, state registration and be the policy holder of full auto insurance coverage
- Clean Department of Children and Families and CT state police checks
- Working knowledge of available community resources
- Previous experience working with juvenile justice programs
- Demonstrated cultural competence
- Bi-lingual English/Spanish preferred Responsibilities
- Responsible for maintaining and collecting data for outcome measurements (JRB/ SDE spreadsheet, DMC data).
- Participate in internal and external task forces, committees, and coalitions as assigned (DMC, LIST)
- Provide ongoing support of interns and staff case management duties
- Adhere to agency policies and procedures
- Responsible for adhering to budgetary guidelines including Enhancement funds
- Receive and process referrals, request and obtain additional documentation needed from referring agency and assign cases to case managers
- Conduct regular quality assurance and case reviews with case managers including documentation completion, data entry, and exit surveys
- Attend and facilitate all JRB panel meetings.
- Send bi-weekly and/or monthly case progress updates to each referral source
- Performs intakes with youth
- Maintains a case file on all youth assigned
- Maintains appropriate contact with clients for service delivery
- Maintains regular and timely contact with all involved service providers for monitoring, case-collaboration and follow-up
- Attends and participates/presents in consultation and staff trainings as scheduled by the supervisor
- Provides written and/or verbal reports and/or documentation as required by referral sources, assist the Team Leader as requested in a timely manner
Pay:
$19.00 - $21.00 per hour Expected hours: 40.0 per week
