Client Base Services - Family Resource Coordinator

Client Base Services Program Manager HOURS:
Full-Time position, minimum of 40 hours per week.

JOB SUMMARY
Responsible for comprehensive case management, a strong commitment to the goals of self-sufficiency, and family stability. Assist economically challenged families to become self-sufficient in the following areas; education, employment, income, housing, transportation, and the reduction or elimination of governmental benefit dependency. Assist clients in developing service plans that are specifically tailored to their specific individual needs.

MINIMUM QUALIFICATIONS
Applicants must have education, training or experience demonstrating competence in the following areas: 1. BS in Human Service or 3 years of actual case management/social work experience. 2. Experience in family budgeting and working with families at or below poverty income. 3. Knowledge of record keeping and files management. 4. Knowledge of community links, to establish, locate, and access resources for families. 5. Ability to communicate effectively both verbally and in writing. 6. Experience using Microsoft Word, Word Perfect, Microsoft Excel and Power Point. 7. Must have valid driver's license.
Note:
As a condition of employment. Upon hiring, must complete TB test, and submit a copy of driving record along with proof of vehicle insurance.
DUTIES & RESPONSIBILITIES 1.
Perform comprehensive and accurate assessment of family needs that describes the family goals, responsibilities, timetables and strategies for achieving these goals to become self-sufficient. 2. Prepares monthly statistical reports regarding case activity. Prepares analytical reports on specific cases as requested by superiors. 3. Conducts assessments on an ongoing basis, schedule interviews and appointments, and home visits. 4. Meets monthly in comprehensive case management team meeting and discuss any problems. 5. Completes paperwork accurately, on time and legible. 6. Ensures that information in client's file is complete, neat, organized, accurate and up-to-date. 7. Measures, interviews, and records client's progress on outcome goals monthly. 8. Acts as an advocate for clients as warranted. 9. Responsible for ensuring reports and records are maintained in compliance with funding source requirements. 10. Knowledge and information to access community services and resources. 11. Provides emergency or crisis assistance in areas such as food, housing, utilities, emergency shelter, clothing, medical and transportation. 12. Assist clients to form linkages with mental health and educational state and local service programs i.e., child abuse, substance abuse, domestic violence and children/families with disabilities. 13. Conducts Zoom meetings, FaceTime meetings, text and e-mails as needed to enhance the knowledge of clients and their environments. 14. Conduct home visits when needed to provide comprehensive services to clients that do not have access to transportation, etc. 15. Ability to effectively navigate the Data Base systems. 16. Maintains a professional attitude by abiding with the agencies confidentiality policies. 17. Performs other duties consistent with the goals of the program as deemed necessary by supervisors.
