Job Description

Case Manager (Navigational Coach) Full-Time | Monday-Friday Please note the schedule will be 12:00p-8:00p Are you passionate about helping individuals and families achieve greater stability and success? We're looking for a compassionate Case Manager to provide case management, connect families with community resources, and support participants in reaching their personal, educational, and employment goals. Responsibilities Provide one-on-one case management and develop individualized success plans. Conduct participant intake, eligibility screenings, and ongoing assessments. Connect families with community resources, including housing, employment, education, financial, and wellness services. Monitor participant progress through regular follow-ups and goal tracking. Maintain accurate case notes, documentation, and program data. Collaborate with community partners and represent the organization at outreach events. Support program reporting, surveys, and outcome tracking. Participate in team meetings and cross-functional collaboration. Qualifications Bachelor's degree in Human Services, Social Work, Education, or a related field (Associate's degree with significant related experience may be considered). Two or more years of experience working with individuals or families in a social service, nonprofit, or community-based setting. Strong case management, communication, and organizational skills. Proficiency with Microsoft Office and database/CRM systems. First Aid/CPR certification required within 60 days of hire. Schedule Monday-Friday, 12p-8p
Pay:
$26.00 - $28.50 per hour
