Case Manager Lead Requirements Bachelors' Degree One year of experience Valid Drivers' License Travel is necessary throughout the Brevard County area. Repsonsibilities Case management and the coordination of client services, clients served, and group planning and facilitation. Monitoring the progress of services and to provide follow up services, if needed, after program completion. Responsible for planning, linkage, coordination and advocacy for all necessary services for the youth. Upholds client confidentiality, respect to clients, and performs in a professional manner in all situations. Functions as part of a treatment team and works a minimum of 40 hours a week and some evening work required.
