Job Description

Job Summary We are looking for a compassionate and proactive Outreach Case Manager to join our team. In this role, you'll be the first point of contact with individuals experiencing homelessness who have expressed an interest in receiving housing or shelter services. As an Outreach Case Manager, you will collect accurate data, build trust and rapport with clients, connect them to vital housing services, and collaborate with different departments and providers to ensure successful exits from the program. This is a field-based role that requires resilience and a strong understanding of housing and addiction support systems. It can require long periods of driving and a considerable amount of time outdoors. The ideal candidate will have significant empathy, interpersonal skills, and self-reliance. Duties Engage with unhoused individuals throughout Miami-Dade County and provide resources for housing and supportive services. Complete at least three hours of outreach daily to engage with new and existing clients and visit suggested outreach areas. Deliver case management services in the field, including housing needs assessments, scanning documents, and transporting clients to and from appointments, among other services. Aid in assembling and tracking inventory of hygiene kits delivered during engagement. Maintain accurate and confidential documentation of outreach efforts and client progress. Assist clients in navigating complex systems including healthcare, benefits, and housing. Provide substance use support and connect clients to recovery resources. Perform motivational interviewing to guide individuals toward stabilization. Refer clients to Continuum of Care (CoC) resources and ensure warm hand-offs to service providers. Qualifications Experience in case management, homelessness services, behavioral health, social services, or a related field preferred. Strong communication skills, both written and verbal. Empathy and cultural sensitivity when working with vulnerable populations. Knowledge of housing programs, CoC systems, and eligibility processes. Experience or familiarity with addiction counseling and community resources. Ability to collect and analyze outreach data for referrals and reporting. Highly organized and detail-oriented in case documentation and client follow-up. Spanish language proficiency preferred. Must be able to commute within Miami-Dade County, FL (Required). Valid driver's license and reliable transportation required.
