Case Manager The Case Manager has the primary responsibility of providing case management services for criminal justice impacted individuals. The Case Manager will be assigned specific clients, referred to as a 'caseload' and monitor the service delivery for these clients while they are in the program from intake to discharge. The ideal candidate has direct experience working in correctional facilities, reentry programs, behavioral-health settings, or community-based organizations serving justice-involved populations. The Case Manager must demonstrate professionalism, empathy, cultural sensitivity, sound judgment, and the ability to maintain appropriate boundaries. Primary Duties and Responsibilities Ensures compliance with standard organization, and program contract policies and procedures of their assigned unit. Coordinate daily program activities in accordance with the established schedule. These activities may include Daily Groups and Community meetings; Weekly program curriculum lectures, Life Skills seminars, Caseload groups, and specialized groups focusing on the areas of Anger Management, Relapse Prevention, Job Readiness preparation, and Parenting skills. Conduct initial intake sessions with new clients in which the case manager will orientate the client about the program requirements Conduct Bi-weekly Individual counseling sessions with assigned caseload. These sessions may include the following tasks: completion of a monthly progress report; Monthly Release planning; Goal setting; Crisis Intervention; Behavioral adjustment sessions; Psychological referrals, and Discharge Planning in order to provide recommendations and referral sites for continued programming upon their release. Maintain charts (electronic and hard-copy versions) for assigned client's caseload and ensure all documentation is completed in accordance with established deadlines and program policies. Minimum Requirements
- Bachelor's degree in social work, criminal justice, psychology, counseling, human services, sociology, or a related field or at least two years of case-management or direct-service experience with incarcerated, formerly incarcerated, or otherwise justice-involved populations.
Basic knowledge of case management, and referral services Basic knowledge of the criminal justice system. Good presentation, public speaking, and facilitation skills. Good organizational skills and attention to detail. Effective communication skills with internal and external contacts at all levels. Ability to handle multiple priorities simultaneously. Problem solving ability. Good interpersonal skills. Knowledge of computers and word processing software such as MS Word, Excel, and Office. Internet navigation skills are required.
- Ability to travel locally and work occasional evenings or weekends as program needs require.
- Valid driver's license and reliable transportation, if travel is required.
- Ability to satisfy applicable correctional-facility clearance and background-screening requirements.

$60,000.00 per year
