Job Description
Our client, a faith-based nonprofit supporting individuals transitioning from incarceration, is seeking a Case Manager to provide ongoing support to program participants as they re-enter society. This role focuses on building meaningful relationships, assessing individual needs, and developing personalized plans of care to guide each client's success. The Case Manager partners with community organizations, connects clients to critical resources, monitors progress, and maintains consistent engagement through one-on-one interactions, all while documenting services and outcomes to support long-term impact. Job Purpose Case Management provides services to clients who are admitted into the client's inspHire Program. The Case Manager plans and organizes collaborative services and develops and administers a Plan of Care for each client, to effectively assist the client with re-entering into society. The Case Manager achieves consistent interactions with clients which include the distribution of services, phone calls and one-to-one sessions for up to a year. Network Partners with Florida Department of Corrections (FDC) to recruit potential clients from correctional facilities, transitional housing facilities, Halfway Houses, and Work Release Facilities. Schedules events or attends community events to raise program awareness and gauge community interest. Weekly, visit FDC supervision offices and local re-entry correctional institutes to present the organization's inspHire Program and collect information on potential clients. Casework The Case Manager uses a variety of techniques to determine clients' needs, subsequently completing individual Plan of Care assessments and interviews. Based on the outcomes of implemented and monitored treatment plans, makes independent professional recommendations that have a distinct effect on the client's reentry opportunities. Maintains or exceeds the organization's established standards for program performance relative to each client's growth and development through program participation. Resourcefully manages the client's professional and personal success. Community Partnership Communicates, connects with, and collaborates with community agencies to offer program clients a wide range of resources. Interprets available resources and skillfully conveys information to clients, ensuring they receive, embrace, and begin their best program experience. Follow-up with clients and agencies as appropriate to document use/referral success. Administrative Aligned with organizational Guidelines to use professional verbiage, the Case Manager meticulously enters all client interactions and transactions into the organization's Efforts-to-Outcomes (ETO) data system. •Any other tasks assigned by inspHire leadership team.
