SUMMARY Responsible for providing

HIV/AIDS

referral for healthcare services to an assigned caseload in accordance with Federal legislation, the Ryan White Treatment Modernization Act of 2009, and the Orlando EMA HIV Health Services Planning Council. These services help consumers gain access to needed medical, social, educational, and other services. This includes direct assistance in gaining access to services, coordination of care, and linkage to appropriate services.

Referral for health care includes:

(1) annual assessment of the eligible consumer to determine service needs; (2) semi-annual re-assessment of the eligible consumer to determine service needs; (3) referral and related activities to help the consumer obtain needed services; (4) monitoring and follow-up; and (5) evaluation.

ESSENTIAL DUTIES AND RESPONSIBILITIES

include the following. Establish rapport with client, assess and determine client eligibility for medical, dental, social, educational, and other services. Perform a needs assessment and document provision of services. Provide support and assistance to clients and their families to assure continuity of care. Make appropriate referrals to Hope and Help Center's services and other social or professional service agencies. Perform oversight of the entire case and linkage to appropriate services. Monitor and follow-up with clients as appropriate. Provide ongoing client monitoring and evaluation of client progress toward self-sufficiency. Maintain accurate, same day records of each client interaction via progress notes and corresponding documentation in the client file. Prepare daily progress notes and meet all other reporting deadlines specific to grantee guidelines. Coordinate with Medical Case Managers for all clients on shared caseloads.

OTHER RESPONSIBILITIES

Performs other work-related duties and special projects as assigned.

EXPERIENCE

Bachelor's degree from an accredited college or university or an associate degree and one (1) year of HIV or health education experience or a high school diploma or equivalent and two (2) years of HIV or health education experience. CERTIFICATES, LICENSES, and/or

Valid Florida Driver's License required.
