About Lumen Lumen provides person-centered supportive living and transitional services designed to empower individuals as they build independence, stability, and wellness. Our mission is to offer a structured, compassionate environment to neurodivergent young adults that promotes personal growth and helps each client achieve sustainable success. Position Summary The Case Manager plays a vital role in supporting clients through individualized service coordination, advocacy, and ongoing engagement. This position ensures that clients' needs are assessed holistically and that they receive the resources, referrals, and support necessary to reach their personal and program goals. Key Responsibilities Conduct comprehensive assessments to identify client needs, strengths, and goals. Develop and implement individualized service plans tailored to each client's needs and objectives. Provide case management and coordination of care, ensuring clients have access to community resources, healthcare, and social supports. Facilitate regular meetings with clients to monitor progress and adjust plans as needed. Collaborate closely with the Lumen team, including clinical staff, residential coaches, and program leadership, to ensure a consistent and integrated approach to care. Maintain accurate, timely, and confidential documentation in accordance with organizational and regulatory standards. Advocate for clients to ensure equitable access to services and opportunities. Participate in ongoing training, supervision, and professional development. Qualifications Master's degree in Social Work, Counseling, Psychology, or a related field (required). Prior experience in case management, social services, or behavioral health preferred. Strong interpersonal, communication, and organizational skills. Ability to maintain professional boundaries while building supportive client relationships. Commitment to Launch with Lumen's mission, values, and trauma-informed approach. Proficiency in documentation and time management within a fast-paced environment. Work Environment Combination of in-office, residential, and community-based work. May require flexible scheduling, including occasional evenings or weekends, based on client and program needs.
