Responsibilities Outpatient Services Step Down Coordinator Partial Hospitalization Programs Peachford Hospital is a 246-bed, acute care psychiatric hospital located in the north-Atlanta Dunwoody neighborhood. Peachford features individual units for children, adolescents, adults, and seniors, and offers inpatient acute care, partial hospitalization, and intensive outpatient programs. On average, over 10,000 patients receive care from our compassionate health care team each year and 500+ outstanding employees call Peachford home. Due to the continued growth in our outpatient services programs, we are searching for a Step-Down Coordinator to guide patients along their journey from inpatient to outpatient care. The PHP/IOP Step-Down Coordinator is responsible for meeting with all eligible patients to discuss the benefits of Peachford's outpatient services as part of their continuum of care treatment. The Step-Down Coordinator will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to outpatient eligibility, into Midas for trending report purposes. A successful candidate will have a thorough understanding of the step-down processes and ensuring patients have a good understanding of the benefits of outpatient services. A thorough understanding of the Inpatient workflow process, as it pertains to case management and discharge planning, is preferred.
Qualifications Education:
Bachelor's Degree required. Mental health/behavioral sciences field or equivalent preferred.
Experience:
A minimum of three (3) years' experience in a healthcare setting or managed care company, with experience in patient assessment, treatment planning, utilization management, and/or case management.
