Job Description

Do you enjoy helping others and are seeking a challenging yet rewarding career? Then Union Mission has the position for you! Union Mission is currently seeking 2 Case Managers (1-Part-Time; 1- Full-Time) within our Emergency Services Department.
The schedules are as follows:
Part-Time:
Tuesday through Saturday - 4:00 p.m. - 9:00 p.m. ___________________________________________________________________________________
QUALIFICATIONS
Bachelors Degree in Social Work or Human Services with at least five years of experience in a social service or clinical setting, or a combination of education and related experience. Skilled in crisis management and documentation. Skilled in trauma-informed care. Ability to establish and maintain relationships with community partners. Must be compassionate. Excellent customer service skills. Good oral and written communication skills. Knowledge of substance abuse, mental health, and housing assistance. Must be proficient in Microsoft Office suite products with ability to learn and use the required HMIS program.
FUNCTION AND PURPOSE OF POSITION
The Case Manager is primarily responsible for engaging with homeless adults, intaking clients after-hours, and providing services, linkages, and assistance with daily activities as needed. The following list of responsibilities is not meant to be all-inclusive and may be adjusted to meet program needs. Provide case management and complete assessments for UMI program clients. Enter data and services into the Client Track System Represent the organization appropriately and provide case management for all UMI program clients professionally in the community Serves as consumer advocate and attend treatment team meetings Analyze data and prepare monthly reports Make appropriate referrals to other providers as well as other community agencies as necessary to minimize crisis and maximize self-sufficiency Assists clients referred by other service providers with social service needs Advocate for clients with community property owners and landlords Coordinate programs specific to the prevention and treatment of alcohol and drug abuse.
