Position Summary CSAH is seeking a compassionate and committed Case Management Outreach Team Member to join our frontline efforts in engaging individuals experiencing unsheltered homelessness. This role focuses on street outreach, relationship-building, and connecting people to critical services, including housing, healthcare, and behavioral health support. In addition to direct outreach, this position plays a key role in supporting the Triage Center at Union Mission, helping individuals stabilize and transition into appropriate housing and services. The Outreach Team Member will also contribute to the implementation of the evolving decentralized Coordinated Entry (CE) system, working in close partnership with the Assistant Director of CE to ensure that outreach efforts are aligned with system-wide goals of accessibility, equity, and community responsiveness. Key Responsibilities Street Outreach & Engagement Conduct regular outreach in targeted areas to identify and engage individuals experiencing unsheltered homelessness. Build trust and rapport through consistent, respectful, and trauma-informed interactions. Provide on-the-spot assessments and referrals to emergency shelter, medical care, behavioral health services, and housing programs. Case Management & Service Navigation Develop individualized service plans in collaboration with clients, focusing on housing stability and wellness. Assist clients in obtaining identification, benefits, transportation, and other essential resources. Maintain regular contact with clients to monitor progress and adjust plans as needed. Triage Center Support Welcome and support individuals accessing the Triage Center at Union Mission. Conduct intake assessments and coordinate with internal and external partners to facilitate service connections. Help clients transition from the Triage Center into bridge housing, permanent housing, or other appropriate placements. Support for Decentralized Coordinated Entry Collaborate with the Assistant Director of CE to integrate outreach efforts into the decentralized CE system. Ensure timely and accurate referrals through CE access points and maintain documentation in alignment with CE protocols. Participate in training and system development activities to strengthen CE accessibility and equity for unsheltered individuals. Collaboration & Advocacy Work closely with the Housing Director, outreach team, and community partners to ensure coordinated care. Advocate for clients within service systems and help reduce barriers to access. Participate in case conferencing, community meetings, and collaborative outreach efforts. Documentation & Professionalism Maintain accurate, timely, and confidential case notes and service records. Uphold CSAH's standards of professionalism, client confidentiality, and brand integrity in all interactions. Ensure all outreach and case management activities reflect the organization's commitment to equity, accessibility, and housing justice. Qualifications Bachelor's degree in Social Work, Human Services, Psychology, or related field (or equivalent experience). Experience in street outreach, case management, or working with unhoused populations preferred. Strong interpersonal skills and ability to build trust with individuals from diverse backgrounds. Knowledge of trauma-informed care, harm reduction, and housing-first principles. Ability to work independently in the field and collaboratively within a team. Commitment to equity, inclusion, and culturally responsive service delivery. Valid driver's license and reliable transportation required. Preferred Qualifications Familiarity with Savannah's homeless services landscape. Experience working within or supporting Coordinated Entry systems and HMIS. Bilingual or multilingual abilities.
