We are often the organization others look to when it comes to ending and preventing homelessness. From emergency shelters and housing programs to health services, employment assistance, and street outreach, we're looking for caring and committed people to join our team. What You'll Actually Do As a Case Manager, you'll be the steady, skilled presence guiding people from crisis to stability.
You'll:
Own a caseload and guide clients through assessment, service planning, and referrals, always centering their voice and choice in the process Connect people to what they need whether its substance abuse and mental health treatment, transportation, and coordinated care with partner providers Show up in crisis moments from conducting risk assessments, coordinating crisis plans with psychiatric providers, and linking clients to hospitals and crisis teams when it matters most Keep meticulous, compliant records that protect your clients and strengthen our programs What You Bring A high school diploma or GED (bonus points for a Bachelor's degree) Ideally 1-2 years of case management or supportive services experience with homeless or underserved populations A working knowledge of homelessness, mental health, and substance abuse issues Sharp time-management skills so you can successfully manage a caseload and help your clients efficiently Communication skills built for conflict resolution and motivating conversations Integrity, patience, and a genuine commitment to the mission Comfort with ambiguity and a fast-moving environment A valid driver's license and a clean traffic record Why This Role, Why Us Real impact, every single day. Schedule:
