The Service Coordinator is responsible for providing support and case management to participants in Humility Homes & Services, Supportive Services for Veteran Families Program(SSVF). The primary focus is assisting Veterans who are homeless, or at risk of losing their housing through case management and assistance with navigating community resources for rental assistance, applying for supplemental benefits such as: FIP, SNAP, WIC, LIHEAP, SSI and Healthcare benefits. The goal is to help the Veteran regain self-sufficiency and maintain permanent housing by connecting them to eligible mainstream resources and other supports. This position works collaboratively and alongside the Veteran's Administration in ensuring the veteran is receiving support to transition from homelessness to housing.
Qualifications:
Education Required:
Bachelor's Degree, with major in Social Services, or related fields, and preferred minimum of 2 years' experience in social services. Four years of verifiable case management in Social Services or Housing Navigation experience, may be substituted for the educational requirement. Must have valid US Driver's License.
Experience:
Individual must possess demonstrated experience with crisis intervention, Trauma-Informed Care, and social work practice with chronically homeless individuals and/or individuals with disabilities (mental illness, substance abuse/addiction, physical disability, developmental disability, chronic health problems, etc.) Individual must also be capable of preserving the confidentiality of employee and participant records. Preferred experience working with people or families who have experienced homelessness and have disabilities of long duration including, mental health, substance abuse, mental or physical impairment Individual must have a results-oriented background and the ability to think strategically, anticipate future trends in homeless services and ability to implement them. Must demonstrate an exceptional capacity for multi-tasking. Individual enjoys working hard and looks for challenges; able to act and react as necessary. Humility Homes and Services, Inc. Responsibilities:
1.Commitment to Ethical and Effective Services Know and implement HHSI's mission, policies, and procedures; and abide by all city, state, and federal laws, and regulations. Maintain training in suicide risk assessment, mandatory child and dependent adult abuse and neglect reporting, crisis intervention, civil rights, housing discrimination, harm reduction, trauma informed care, housing first practices, boundaries, bloodborne pathogens, fire safety, cultural competency, workplace safety, active shooter, CPR/First. Aid, Verbal Judo, Trauma Informed Care, domestic violence training and Housing First. Collaborate with other VA Service providers to ensure proper placement of Veterans into programing. Maintains current knowledge of evidence-based practices as they relate specifically to serving veterans, individuals and families of whom are experiencing homelessness or housing instability. Case Management:
Complete required training to obtain and keep HMIS license, including state required Coordinated Entry training. Maintain access to Squares, regularly review the VA SSVF Program Guide, to stay current on program changes and required VA training such as S.A.V.E Suicide prevention training. Conduct comprehensive screenings and assessments to determine program eligibility. Identify housing barriers, strengths, and immediate needs, during the Intake and Assessment process. Provide housing stability guidance through; collaborating with veterans to create and update personalized Housing Stability Plans, aimed at achieving long-term self-sufficiency. Creates Individualized Service Plans with Veterans based on goals set by Veteran. Meets with Veteran regularly, to provide assistance in goal achievement, frequency and location of meetings are determined by program requirements and individual needs, however must not be less than once monthly. Completes housing needs and mainstream resources assessment with Veterans. Connects Veterans with; VA benefits, public assistance (SNAP, SSI/SSDI), healthcare, mental health counseling, substance use support, and legal services; assists Veterans in applying for services when applicable. Regularly evaluate the needs of participants on caseload and provide them with ongoing support. Complete any other mandated assessments required by the grant agreement or program policy and procedures. Process and submit Temporary Financial Assistance (TFA) requests for; rental arrears, security deposits, utility payments and all other allowable expenditures. Complete and submits Promise to Pay letters as necessary, to secure housing and utility services. Reviews all previous TFA request for Veterans to ensure all expenditures are allowable to prevent overpayments. Completes a budget with Veterans at entry, and program recertifications. Assists Veterans with adhering to budget, by assisting with paying bills, grocery shopping, and prioritizing needs over wants. Transport Veteran to local food pantries and HHSI's Corner Closet Donation Center, as needed for hygiene, clothing and household needs. Provides Life Skills training such as cleaning, laundry, navigating the bus system etc. for program Veterans. Works with appropriate Veteran's Administration staff to meet the needs of the veterans, whether the veteran is experiencing homelessness, or needing homeless prevention funding. Provides crisis intervention and advocacy for Veterans, as needed. Transports Veterans to appointments, apartment showings and any other necessary locations based on their ISP. Ensure Veterans understand program guidelines and Participant Rights and Responsibilities. 3.
Documentation:
Responsibilities for Data Management include: ensuring accurate, contemporaneous client notes and data are entered into the Homeless Management Information System (HMIS) in compliance with program standards. Completes daily case notes, service transactions, and updates of veteran demographics by entering data into ServicePoint within required timeframes. Maintains current income verification, eligibility and services provided as required for specific grant agreement. Maintains releases of information for all participants and external providers when applicable. Refers participants to other programs at HHSI for which they are eligible, along with supportive documentation. Assists Veteran with obtaining documents such as ID, SS Card, birth certificate and proof of income. Accurately tracks TFA provided in Docuware, case notes and TFA sheet in participant file. Enters accurate data into Service point within the required time frames, closes files in Service point within 48 hours of exiting Veteran from programing. Provides Veterans with required exit surveys. 4.
Other Duties:
Participate in Outreach efforts, including; participating in community outreach events, coordinating with local shelters or Coordinated Entry systems to locate eligible veterans. Mandatory participation required in the Annual Winter and Summer PIT counts, to locate Veterans. Collaborates on case plans for high-need Veterans with the Supervisor. Attends mandatory VA conferences and training, as assigned by Supervisor/Chief of Programs; including out-of-state conferences/training. Case Conferences weekly, with Supervisor and other VA Service Providers. Attends mandatory Agency training/meetings. Maintains knowledge of Evidenced Based Practices for specific program. Maintains strong knowledge of community resources, especially as it relates to housing. Regularly promotes the agencies' mission and advocates for social justice. Cross trained to cover the 1st shift Emergency Shelter Monitor position. Performs other duties, as needed
Schedule:
Full-Time. Must be able to work 36 hours per week, this is an in-office position with work in the field required. General hours are Monday through Friday between 7am and 5pm; however, must be flexible for earlier days or later nights as needed to connect with participants
Salary:
$18.10-$24.37 per hour. This is an hourly, non-exempt position . Starting wages are determined strictly by independent, market-based compensation reports relative to individual qualifications, and are non-negotiable. Benefits eligible
