Job Description

Schedule:

Monday - Friday 8:00 am - 4:30 pm

Description :

provide the following: Community Support Services Counseling Case management Crisis Intervention On-going support to adults with mental health issues at Transitions Maintain accurate records regarding all services provided and complete such documentation in an accurate and timely manner Responsible for meeting and/or exceeding billing expectation

Requirements:

Bachelor's degree in human/social services At least 1 year of experience in the field is required Must have a valid driver's license Must have excellent communication and organizational skills Sensitive, patient, and responsive. Transitions Mental Health Services is an equal opportunity employer
