Our Case Manager will act as a liaison among patients, healthcare providers, and other necessary parties, ensuring that both the fiscal and health-related needs of patients are met efficiently and empathetically. Responsibilities Assess patient needs and establish detailed care plans in collaboration with healthcare providers. Regularly evaluate and update patient plans based on medical and financial assessments. Facilitate communications and necessary negotiations between patients' families, healthcare facilities, and financial stakeholders. Ensure compliance with healthcare regulations and financial advisory standards throughout the patient care process.
