In this role, you will be pivotal in coordinating comprehensive care plans for clients within healthcare organizations undergoing financial restructuring and strategic transformation. Our Case Manager will act as a liaison among patients, healthcare providers, and other necessary parties, ensuring that both the fiscal and health-related needs of patients are met efficiently and empathetically. Responsibilities Assess patient needs and establish detailed care plans in collaboration with healthcare providers. Regularly evaluate and update patient plans based on medical and financial assessments. Facilitate communications and necessary negotiations between patients' families, healthcare facilities, and financial stakeholders. Ensure compliance with healthcare regulations and financial advisory standards throughout the patient care process. Manage multiple case loads with precision and adherence to confidentiality agreements. Report and document patient progress and care outcomes for financial analyses and procedural compliance. Required Skills Expertise in patient assessment and case management Strong understanding of healthcare systems and financial impacts Ability to work collaboratively across diverse teams Exceptional organizational and communication skills Proficiency in data management and reporting tools such as Electronic Health Records (EHR)
