Overview Join our dedicated team as a Case Manager specializing in Corrections, where you will play a vital role in coordinating comprehensive care for the population served. Your expertise will help identify risk/needs, and ensuring they are compliant with any legal requirements. This dynamic role offers an opportunity to make a meaningful impact by advocating for vulnerable populations. If you are energetic, detail-oriented, and passionate about patient-centered care, this position is perfect for you! Responsibilities Conduct thorough risk assessments to evaluate risk/needs. Develop individualized case plans Coordinate services and resources. Manage processes such as intake, assessments, discharge planning to optimize resource use. Collaborate with stakeholders, and community partners Maintain strict compliance while handling sensitive records and information. Skills Proven experience in corrections environments, case management, and data entry. Strong knowledge of criminal justice systems, and/or correctional settings Ability to perform comprehensive assessments with attention to detail Excellent communication skills. Background in social work or mental health counseling is highly desirable for supporting behavioral health needs.
