Job Description

Position:
Case Manager Regional Health Systems is Northwest Indiana's largest provider of community-based mental health care, primary health care and addiction treatment services. We offer adults and children—both uninsured and insured—integrated care through our community mental health center and federally qualified health center. Regional Health Systems is a member of the Regional Care Group .
Position Purpose :
To provide case management and ADL services to SMI, CA and SED clients and their families.
These services are directed toward:
providing the skills and supports necessary to enable clients to function in the least restrictive environment with the highest possible level of functioning. Case managers provide assessment, service planning, coordination and linkage and monitoring functions. Population to include client community commitment under the supervision of the Civil Court. Working with clients who have been released from jail/prison that may also be on parole. Clients may have mental diagnoses.
Essential Function:
Intake/Assessment Treatment planning Case Management/ADL Coordination of Services Communicate with referral sources such as Civil Court Judge and Court Liaisons Crisis Intervention Clinical Documentation Financial Risk Assessment Transports clients as required safely and on time, if applicable Revenue-Productivity Policies, Procedures, Manuals, Resource Documents Ethical/Legal Issues•understanding of state codes and following standards Diversity Computer Literacy Teamwork and performance improvement Communicates effectively Works cooperatively with supervisor and others

East Chicago, IN Requirements:
Bachelor's degree in Social Work, Psychology, or related clinical field One year of experience with the target population (adult & mental health)
Valid Driver's License & Insurance and Clean Driving Record Shift Details:
Full-time, 40 hours a week opening•
Non-exempt:
Days and evenings depending on the needs of the department, and may include on-call weekends and holiday.
