8:30 - 5:00 For over 60 years, Public Advocates in Community Re-Entry (PACE) has provided resources and services to assist justice-involved individuals and their families to lead productive and responsible lives in our community. Our team believes that felonies don't define futures , and that no one's life should be defined by their worst day. To support successful re-entry, PACE works to reduce recidivism. Job Summary The Recovery Case Manager plays a vital role in supporting clients throughout their recovery or behavioral health treatment journey. This position is responsible for coordinating a comprehensive continuum of care, providing resource linkage, managing logistics, and advocating for clients to ensure long-term stability and sustained recovery. Acting as a central navigator, the Recovery Case Manager removes structural barriers to recovery by assisting with housing, employment, and healthcare navigation. Key Responsibilities
    Intake & Assessment:
    Conduct comprehensive assessment to identify the client's strengths, resource deficits, medical requirements, and structural barriers to recovery.
      Care Planning:
      Develop, implement, and track individualized service plans (ISPs) in close collaboration with the client and the multidisciplinary clinical team.
        Resource Linkage:
        Connect clients to critical community resources, including stable housing, medical, mental health services, transportation, and childcare.
          Income Support:
          Assist clients with navigating legal or entitlement paperwork (FMLA, SNAP, SSI/SSD).
            Advocacy & Liaison:
            Serve as the primary liaison between the client and external entities such as landlords, probation officers, legal representatives, and family members.
              Crisis Intervention:
              Utilize trauma-informed care and de-escalation techniques to manage behavioral crises and provide recovery and harm reduction support.
                Care-Coordination:
                Coordinate proactive transition plans ensuring placement in sober living environments, booking travel, and aligning outpatient counseling.
                  Compliance & Documentation:
                  Maintain highly accurate, timely, and confidential client records in the case management database, Efforts to Outcomes (ETO). Joint Position Responsibilities
                  • Helps navigate recovery support systems.
                  • Provides clients with ongoing education regarding substance abuse, mental health, and other health and wellness information.
                  • Finds creative and effective solutions to complex problems and justifies prioritization when needed.
                  • Make follow-up calls after all referrals are made. Ensure all referrals are documented.
                  • Ability to facilitate recovery support groups as needed and addiction education groups.
                  • Ensure communication is made with the criminal justice monitoring agency (i.e. probation/parole/community corrections officers, etc.) to inform them of services PACE provides and recommendations on any additional service needed.
                  • Provide case notes for every intervention (i.e., appointment, phone call, etc.) in the client services database (ETO). Maintain confidentiality and standards of ethical practice.
                  • Addresses barriers to successful re-entry and recovery and serves as a case manager and advocate.
                  • Advocate to ensure delivery of necessary services.
                  • All other duties as assigned. Minimum Qualifications
                  Education:
                  Bachelor's degree in social work, Psychology, Counseling, Human Services, or a related behavioral health field preferred. High School Diploma or equivalent required.
                    Experience:
                    Minimum of 1-2 years of direct experience in recovery support and case management.
                      Knowledge Base:
                      Strong foundational understanding of addiction behavior, trauma-informed care practices, regional social safety-net resources, and the recovery ecosystem. Essential Skills & Attributes
                        Communication:
                        Exceptional active listening and interpersonal skills to build rapport while maintaining strict professional boundaries.
                          Organization:
                          High proficiency in time management, case tracking, and managing a caseload of 15-20 clients simultaneously.
                            Technical Literacy:
                            Intermediate proficiency with standard office software (Microsoft Office Suite) and experience navigating electronic database systems.
                              Emotional Intelligence:
                              Strong conflict resolution abilities, patience, and a non-judgmental, empathetic approach to human services.
