In this vital role, you will coordinate and oversee case management services, ensuring clients receive comprehensive support tailored to their unique needs. Your energetic approach will help foster positive outcomes by connecting clients with essential social services, behavioral health resources, and workforce development programs. You will serve as a leader within your community, advocating for high-risk populations and supporting their journey toward stability and independence. Duties Lead case management efforts by developing personalized plans that address clients' behavioral health, social service, and workforce needs Coordinate with community partners to facilitate access to addiction counseling, mental health services, and disability support programs Conduct client assessments using motivational interviewing techniques to identify barriers and strengths Manage data entry and maintain accurate client records using Microsoft Office tools and specialized case management software Provide crisis management support by responding promptly to urgent client needs and coordinating appropriate interventions Deliver culturally competent customer service, ensuring respectful communication with diverse populations Educate clients on available resources such as Section 8 housing assistance, liability insurance options, and social services programs Support high-risk populations through targeted outreach, advocacy, and ongoing engagement strategies Document all interactions thoroughly while maintaining confidentiality and compliance with privacy regulations Experience Proven experience working with individuals from underserved communities or high-risk populations in a social work or case management capacity Strong knowledge of behavioral health principles, addiction counseling techniques, and crisis intervention strategies Familiarity with social services systems including Section 8 housing, disability programs, and workforce development initiatives Proficiency in Microsoft Office Suite (Word, Excel, Outlook) for documentation and reporting purposes Excellent client communication skills with the ability to motivate change through motivational interviewing methods Experience working with culturally diverse communities to provide inclusive support services Able to effectively communicate in an oral and written format to professionals and non-professionals Bachelors Degree Strong organizational skills and the ability to track progress on cases in several dozen counties concurrently
