In this vital role, you will coordinate and oversee comprehensive end-of-life care for patients in their homes, hospice facilities, or outpatient settings. Your expertise will ensure that patients receive personalized, high-quality support that aligns with their medical needs and personal wishes. As a key member of our hospice care team, you will facilitate communication among patients, families, physicians, and interdisciplinary staff to promote comfort, dignity, and quality of life during this sensitive time. Duties Conduct thorough patient assessments to evaluate physical, emotional, and psychosocial needs, utilizing your knowledge of physiology and medical terminology. Develop individualized care plans in collaboration with physicians, nurses, social workers, and other healthcare professionals to ensure holistic patient support. Manage discharge planning and coordinate post-acute care services to facilitate smooth transitions between care settings. Provide case management by monitoring patient progress, adjusting care plans as needed, and ensuring compliance with HIPAA regulations and healthcare standards. Educate patients and families on disease processes, symptom management, medication administration, and advance directives to empower informed decision-making. Document all patient interactions accurately in medical records and ensure timely updates for continuity of care. Facilitate diagnostic evaluations and coordinate outpatient or inpatient services to meet evolving patient needs effectively. Requirements Knowledge of infectious disease care, behavioral health issues, geriatrics, pediatrics (PICU), or medical-surgical nursing is highly desirable. Demonstrated ability to perform comprehensive patient assessments including vital signs and physiological evaluations. Proficiency in medical terminology, medical records management, and understanding of HIPAA privacy regulations. Experience in intake processes, diagnostic evaluation coordination, case management activities, and inpatient/outpatient services is preferred. Excellent communication skills with the ability to collaborate effectively across multidisciplinary teams while maintaining a compassionate approach for patients facing end-of-life challenges.
