Purpose:
Provide case management, resources, assessments and support to Independent Living Youth who have chosen recommitment (aged 18-21) in Northern Kentucky. Lead the ILP program within the continuum of Youth Housing. Shift work may include time on the on-call rotation and support across Youth Services.Job Responsibilities:
Work collaboratively with local agencies for connections and resources. Support youth to independence and self-sufficiency through case management services. Maintain Compliance with funders and the state of Kentucky to ensure that all documentation, monthly reports and semi-annual reports are completed and accurate and address the outcomes identified. Coordination of services, including documentation, to serve the needs of the youth. Partner with Brighton Center's Youth, Housing and other programs to support youth to self-sufficiency. A minimum of one in-home visit each week and additional visits as needed will have to be conducted.Requirements:
