Job Description
Job Summary:
We are seeking a dedicated and compassionate Case Manager to join our team at Tatyanas Case Management under the Medicaid HCB Waiver Program. The Case Manager will be responsible for assessing client needs, developing plans of care, coordinating services, and advocating for clients within the program. The ideal candidate should have strong communication skills, knowledge of community resources, time management skills, a proactive approach to problem-solving, the ability to multitask effectively in a fast-paced environment, and experience in case management or social services.Responsibilities:
Meeting/contacting clients on a regular basis to ensure their safety and well being. Establishing, maintaining, and managing effective relationships with Providers. Conduct initial assessments and ongoing reassessments of clients to determine eligibility and service needs within the Medicaid Home and Community Based Waiver Program. Develop and implement person-centered care plans in collaboration with clients, their families, and interdisciplinary teams. Coordinate and monitor the delivery of services and supports outlined in plans of care, ensuring they meet the needs and preferences of clients. Advocate for clients to ensure they receive appropriate services, resources, and support. Maintain accurate and up-to-date documentation and records in accordance with program standards and regulatory requirements. Provide education and guidance to clients and their families regarding available services, community resources, and program guidelines. Collaborate with healthcare providers, community agencies, and other stakeholders to facilitate access to needed services and supports. Participate in team meetings, case conferences, and training sessions as required. Monitor and evaluate client progress and adjust care plans as needed to promote positive outcomes. Helping to manage patient health care needs and services in areas such as long-term care, mental health, substance abuse, and geriatric care. Ensure compliance in accordance with relevant regulations, health care best practices, and internal policy and procedure requirements.Requirements:
Bachelor's degree in Social Work, Psychology, Human Services, or a related field required. Master's degree preferred. Previous experience in case management, social services, or a related field preferred. Experience working with clients to develop a treatment plan. Knowledge of Medicaid and Home and Community Based Waiver Programs is advantageous. Strong assessment, problem-solving, and communication skills. Ability to work independently and as part of a team in a fast-paced environment. Background in health care, social work, mental health, disability care, substance abuse recovery, geriatric care, and long-term care. Proficiency in computer skills, including MS Office and case management software. Valid driver's license.Preferred Qualifications:
Certification or licensure as a Social Worker or related field in the state of Kentucky is beneficial. Experience working with diverse populations and understanding of cultural competency. Familiarity with Kentucky's Medicaid waiver programs and services.Working Conditions:
This position operates primarily remotely with some in office needs, but may require occasional travel to client homes, healthcare facilities, or community locations. Flexible hours may be necessary to accommodate client needs and program requirements. Pay Range $70 - $100 Per case.Job Type:
