Responsibilities:

Coordination of Services :

Facilitate the coordination of care by linking the client to various services, including medical, social, educational, and other support services.

Monitoring and Follow-Up :

Continuously monitor the client's progress and the effectiveness of the services provided, and make necessary adjustments to the care plan.

Advocacy and Support :

Act as an advocate for the client, providing support and resources to help them navigate the complex system of services. Collaboration with

Work as part of a treatment/service team, often taking the lead role, and using discretion and independent judgment to promote recovery.
