The ideal candidate will possess a strong background in patient care and case management, with experience in various settings. This role involves coordinating patient care, ensuring appropriate resources are utilized, and advocating for patients' needs throughout their time in the program.
MUST HAVE TARGETED CASE MANAGER CERTIFICATION.
Responsibilities:
Development of Individualized Care Plans :
Create and implement individualized care plans that address the client's specific needs and goals.Coordination of Services :
Facilitate the coordination of care by linking the client to various services, including medical, social, educational, and other support services.Monitoring and Follow-Up :
Continuously monitor the client's progress and the effectiveness of the services provided, and make necessary adjustments to the care plan.Advocacy and Support :
Act as an advocate for the client, providing support and resources to help them navigate the complex system of services. Collaboration withTreatment Teams :
Work as part of a treatment/service team, often taking the lead role, and using discretion and independent judgment to promote recovery.Pay:
