The Targeted Case Manager will work with a Licensed Clinical Professional (LPCC, LCSW, etc.) and will carry a caseload that coordinates, plans, assesses, and advocates for patients to help meet practical needs and improve quality of life by finding cost-effective resources in the community. We are looking for a positive team player who will promote a culture of inclusion, participation, and unconditional, positive regard toward all participants in our long-term recovery support program.

Case Manager Responsibilities:

Assess client needs and work with the client to develop a Person-Centered Treatment goal Minimum of 4 contacts monthly, with at least 2 face-to-face client contacts Meet with the Case Management Supervisor twice per month to discuss cases Complete Pre-Authorizations efficiently and stay in contact with MCO's about client care Refer clients to resources in the community to help them meet treatment goals Document encounters with clients by Kentucky State regulations Education/Background Bachelor of arts or science degree in a human services/behavioral science field with one-year post-educational work experience working with adults in a human service setting Successfully complete a company-approved targeted case management training within six months of employment as a Targeted Case Manager Skills Required Strong organization, computer, and time management skills Ability to communicate effectively, both verbal and written Ability to work both independently and with a team
