Job Description

Position Overview:
We are seeking a dedicated and motivated Targeted Case Manager to provide support services to individuals with mental health and substance use needs. This role involves working directly with clients referred through the court system and external providers, helping them access resources, maintain compliance with treatment plans, and improve overall stability.
Key Responsibilities:
Receive and engage clients referred from court systems and community providers Attend court proceedings as needed to support and advocate for clients Conduct comprehensive assessments Coordinate care with mental health providers, substance use treatment programs, and other community resources Monitor client progress and ensure adherence to court requirements and treatment goals Maintain accurate and timely documentation in compliance with regulations Build relationships with local courts, attorneys, and referral sources
Qualifications:
Bachelor's degree in Social Work, Psychology, Human Services, or related field (required) Experience working with mental health and/or substance use populations Knowledge of community resources and court systems preferred Strong organizational, communication, and advocacy skills Ability to work independently and manage a flexible schedule Reliable transportation and willingness to travel locally
Work Environment & Schedule:
Flexible, self-managed schedule Combination of court appearances, fieldwork, and remote documentation Contract-based position with pay per client served + additional incentive for each new client gained.
