Job Description

Role:
Case Management Supervisor Reports To:
Program Manager Responsibilities:
Responsible for monitoring and evaluating the case management program. Coordinate various aspects of case management services including referral, intake, eligibility determination, program planning, monitoring, assessment, and evaluation of consumer needs / services. Train and supervise case management staff. Prepare reports, keep data, and present information relative to Case Management activities. Serve as advocate for the consumer and facilitate the case management team in the participation of the Community Action Network.
Qualifications:
Master's Degree in Social Service related field with 2 years' experience working with children preferably in Maternal Child Health and at least 1 year of experience providing direct supervision or Bachelor's Degree in Social Service related field with 10 years' experience working with children preferably in Maternal Child Health with at least 2 years' experience providing direct supervision. Must have a valid driver's license and reliable transportation, and appropriate up to date vehicle insurance.
