Job Description

Southwind Healthcare & Rehabilitation, a person centered, long term care facility, is now seeking a Healthcare Professional to serve as our Resident Case Manager. This is a Monday through Friday, 8am to 4pm position. The main objective of this position is to assist our admissions and billing office team to facilitate a pleasant and efficient transition for admitting residents into the facility. Responsibilities and Duties include but are not limited to: Help the Social Services Director (SSD) to handle phone inquiries with the goal of building rapport and scheduling a tour. Work with the Marketing and Admissions Coordinator to keep referral log updated and accurate. Work the SSD to help communicate new referrals immediately to the Marketing Coordinator, Billing & Collections Coordinator, DON, and Administrator. Work with housekeeping and maintenance staff to ensure resident rooms are clean, properly equipped and in good repair prior to admission. Coordinate with facility staff to ensure a pleasant and smooth transistion for the admitting resident and their family. Assist the Business Office Manager to check resident benefits and to help resident obtain Medicaid eligibility, if applicable. Immediately following a resident admission, visit the resident frequently to ensure a pleasant adjustment and that the resident feels a sense of belonging. Qualifications and Skills Must have a genuine interest in the elderly. Must exercise patience and tactfulness. Possess good listening skills. Has sufficient understanding and self-discipline to relate to the patients' needs and feelings. Has an interest in learning and advancement. Possess the ability to work congenially with others.
