Job Description
Case Manager - Adult Family Care (AFC) Access Care Partners $24-$26/hour | Full-Time Make a Difference Every Day At Access Care Partners , our mission is to help older adults and individuals with disabilities live safely, independently, and with dignity. We're seeking a compassionate and organized Case Manager to join our Adult Family Care team and make a meaningful difference in the lives of participants and their caregivers. What You'll Do Conduct participant and caregiver assessments, including home visits. Coordinate care plans with participants, families, caregivers, nurses, and community providers. Provide ongoing case management, support, and resource coordination. Maintain timely, accurate documentation and regulatory compliance. Participate in community outreach and program enrollment activities. Qualifications Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or a related field. Minimum two years of experience working with older adults, individuals with disabilities, or in a human services setting. Strong communication, organizational, and documentation skills. Bilingual candidates encouraged to apply. Schedule & Work Environment Monday-Friday, 9:00 AM-5:00 PM or 8:00am-4:00pm (New hires typically work 9am-5pm to start out for training purposes.) After 6 months of satisfactory performance, employees may be eligible for: up to two days working remotely. Regular community and home visits are required. Requirements Valid driver's license. Reliable personal vehicle available for work-related travel. Ability to conduct in-home visits throughout the community.
