Case Manager -Road to Renewal Community-Based Residential Re-Entry Program Lowell, MA About Us Road to Renewal is a residential reentry program that helps men successfully transition from incarceration back into the community. Through individualized support, case management, recovery services, and community partnerships, we empower residents to build stable, productive, and meaningful lives. Position Summary We are seeking a compassionate, motivated, and organized Case Manager to join our team. The Case Manager will provide direct support to residents by assessing needs, developing individualized service plans, coordinating community resources, monitoring progress, and advocating for successful reintegration into the community. The ideal candidate is passionate about helping individuals overcome barriers related to substance use, mental health, housing, employment, and criminal justice involvement. Essential Duties and Responsibilities Conduct assessments and identify client strengths, needs, and barriers. Develop and oversee individualized reentry plans. Connect residents with housing, employment, healthcare, recovery services, mental health treatment, and other community resources. Maintain regular communication with probation, parole, court personnel, and treatment providers. Facilitate groups focused on recovery, life skills, relapse prevention, anger management, and community reintegration. Provide crisis intervention and support as needed. Complete accurate and timely documentation, case notes, and reports. Monitor resident compliance with program expectations. Assist with medication observation and documentation following agency policies. Support a safe, structured, and recovery-oriented residential environment. Participate in team meetings and collaborate with program staff to ensure quality resident care. Perform other duties as assigned. Qualifications Required Bachelor's degree in Social Work, Psychology, Human Services, Counseling, Criminal Justice, or a related field. At least 1 year of case management, behavioral health, addiction treatment, residential, or human services experience. Strong written and verbal communication skills. Ability to work independently and as part of a team. Valid driver's license and reliable transportation. Ability to pass a CORI/background check and drug screening. Preferred Experience working with justice-involved individuals, reentry populations, or residential programs. Knowledge of substance use disorders, mental health services, and trauma-informed care. Experience with Electronic Health Records (EHR). Bilingual candidates encouraged to apply.
