Lifeworks is hiring a Relief Assistant Case Manager to work with Individuals with ID/DD at our Day Habilitation. We are a nonprofit human service agency that provides adults with developmental disabilities with the opportunity to live engaged and meaningful lives. We're searching for an enthusiastic individual who will bring their personality, an independent work ethic, and a strong belief in enhancing the lives of those we serve. •Candidates must have interest in working or direct experience working with the community we serve.
Essential Duties:
Provide individual and small group instruction in self-care, wellness, activities of daily living, communication, social and motor skills. Provide direct care for ADLs and group activities; Lift and transfer individuals as appropriate to their needs. Communicate respectfully and pro-actively with families, residential staff and all members of participants' teams. Attend in-service trainings as required. Comply with all applicable State and Federal regulations, agency policy and procedures, and personnel practices. Protect the health and safety of all participants and staff. Excellent interpersonal skills. Ability to work in a team environment. Perform other duties as requested by supervisor(s)
Requirements:
Experience working with Individuals with ID/DD High School diploma/GED, related experience preferred. Must be able to pass Fingerprint background check, CORI, DPPC and Driving background checks Valid Driver's License required Excellent verbal and written communication skills Lifeworks, Inc. believes diversity strengthens the value of our workplace and communities.
