As a Case Manager supporting our Individual Supports (IS) and Community-Based Day Services (CBDS) programs, you'll play a vital role in helping individuals achieve personal goals, build relationships, access community resources, and strengthen independent living skills. This flexible, hybrid position floats between our CBDS and Individual Supports programs, providing both direct support and case management services based on individual and program needs. You'll work closely with individuals, families, community providers, and interdisciplinary teams to promote choice, inclusion, and personal growth. ASL fluency is strongly preferred. Schedule Flexible schedule; primarily weekday hours Weekend availability (Saturday and/or Sunday) may be needed
CBDS Program Hours:
Monday-Friday, 8:00 AM
  • 4:00
PM Individual Supports Program Hours:
Sunday-Saturday, 9:00 AM
