Regional Program DirectorPOSITION SUMMARY
Caresify Home Care is seeking a dependable, organized, and highly motivated Case Manager to support the day-to-day operations of our Baltimore, Maryland office. The Case Manager plays a key role in coordinating client services, managing Direct Care Worker (DCW) schedules, monitoring Electronic Visit Verification (EVV), supporting payroll processes, and maintaining effective communication with clients, caregivers, managed care organizations (MCOs), and office staff. This position requires strong organizational skills, attention to detail, professionalism, sound judgment, and the ability to manage multiple priorities in a fast-paced home care environment. The Case Manager will also be cross-trained to provide support for HR and Client Services functions as needed.ESSENTIAL DUTIES AND
RESPONSIBILITIESClient & Caregiver Scheduling Respond to incoming office calls and operational requests related to client schedules and service needs. Coordinate with the Client Service Supervisor to ensure appropriate DCW assignments based on client needs, schedules, and caregiver qualifications. Manage ongoing DCW scheduling, including call-offs, schedule changes, replacements, and coverage needs. Communicate schedule changes promptly and accurately to clients, caregivers, and appropriate office personnel. Assist with maintaining adequate staffing coverage to minimize missed services. EVV & Service Monitoring Monitor Electronic Visit Verification (EVV) activity and identify discrepancies or missing information. Follow up with clients and DCWs regarding EVV discrepancies and provide instructions for correcting or preventing future issues. Review service activity to ensure scheduled services are properly documented. Complete and maintain missed-service reports and follow-up documentation. Escalate recurring or significant service issues to management. Payroll & Administrative Support Collect, review, and process DCW and office employee payroll-related documentation. Review timekeeping and payroll information for accuracy and completeness before submission. Identify and resolve payroll discrepancies in a timely manner. Submit approved payroll documentation to the appropriate payroll administrator or service. Maintain accurate and organized records related to scheduling, payroll, and client services. Communication & Customer Service Maintain professional communication with clients, DCWs, MCO representatives, and Caresify office staff. Address questions, concerns, and operational issues promptly and professionally. Provide excellent customer service while maintaining appropriate professional boundaries. Communicate important client or employee concerns to the appropriate supervisor or department. Maintain accurate documentation of relevant communications and follow-up activities. Compliance & Incident Support Serve as backup support for incident report intake and documentation when needed. Follow Caresify policies and procedures related to client safety, employee compliance, documentation, and service delivery. Maintain strict confidentiality of client and employee information. Protect confidential information in accordance with HIPAA and applicable federal and state regulations. Assist with internal audits, quality assurance activities, and compliance initiatives as assigned. Additional Responsibilities Provide backup support to HR and Client Services as needed. Participate in staff meetings, training, and required agency activities. Assist with special projects and other administrative responsibilities assigned by management. Maintain flexibility to assist with operational needs during periods of increased activity. Be available for on-call coverage, evenings, weekends, or direct care support when required by agency operations.PROFESSIONAL STANDARDS

Required High school diploma or GED. Minimum of one year of experience in home care, healthcare, case management, scheduling, care coordination, or a related field . Strong computer skills, including Microsoft Word and Excel. Excellent written and verbal communication skills. Strong organizational and time-management skills. Ability to prioritize multiple tasks and meet deadlines. Ability to work independently and make appropriate decisions within established policies and procedures. Strong attention to detail and accuracy. Professional customer service and interpersonal skills. Ability to maintain confidentiality and handle sensitive information appropriately. Ability to work effectively in a fast-paced office environment. Preferred Previous home care or personal care services experience. Experience with Electronic Visit Verification (EVV) systems. Experience with caregiver scheduling. Experience with payroll or timekeeping processes. Experience working with managed care organizations or healthcare programs. Previous supervisory, lead, case management, or care coordination experience. Background & Compliance Requirements Employment is contingent upon successful completion and maintenance of all required employment and regulatory clearances, which may include: Criminal background checks. FBI/background clearances as required. Child abuse/neglect clearances, when applicable. OIG/SAM or other required exclusion screenings. TB screening or other health documentation required for the position. Any additional clearances or documentation required by Caresify Home Care or applicable Maryland regulations. Transportation Reliable transportation may be required when business operations require travel between client assignments, office locations, or other work-related locations. Employees who are required to drive for work must maintain a valid driver's license and appropriate automobile insurance in accordance with Caresify Home Care's personnel policies.CORE COMPETENCIES
