PRP Counselor/Case ManagerOverview Enoved & Co. is seeking a dedicated, compassionate, and motivated PRP Counselor/Case Manager to join our team. The ideal candidate is passionate about mental health, committed to supporting individuals in achieving greater independence, and able to build positive relationships with clients, families, schools, community partners, and other members of the treatment team. Key Responsibilities Provide comprehensive case management services to individuals enrolled in the PRP, including assessments, treatment planning, service coordination, and ongoing support. Serve as a liaison between youth participating in the program, their schools, probation officers, families, and other involved professionals. Conduct comprehensive assessments to identify clients' needs, strengths, goals, and barriers to achieving successful outcomes. Develop and implement individualized rehabilitation plans in collaboration with clients, families, and multidisciplinary treatment teams. Facilitate individual sessions and skill-building groups focused on independent living, social skills, coping strategies, community integration, and other areas identified in clients' treatment plans. Coordinate and monitor services to ensure clients receive appropriate and timely care and support. Assist clients in accessing community resources, educational opportunities, healthcare services, and other supports that promote overall well-being and quality of life. Advocate for clients' rights, needs, and access to appropriate services and community resources. Maintain effective communication and collaboration with families, schools, probation officers, providers, and other community stakeholders. Accurately document client progress, interventions, service coordination activities, and outcomes in accordance with program and regulatory requirements. Maintain complete, accurate, and up-to-date client records. Monitor client progress and make appropriate recommendations or adjustments to services as needed. Stay informed about current research, evidence-based practices, industry standards, and applicable regulations related to psychiatric rehabilitation and case management. Qualifications & Skills Bachelor's degree from an accredited college or university, preferably in a health-related or human services field. Candidates with 30 credit hours toward a bachelor's degree may also be considered. Alternatively, an associate degree in a health-related field or at least one year of work experience in a supervised mental health setting . Strong interpersonal and communication skills, with the ability to establish rapport and develop positive, professional relationships with clients, families, and community partners. Excellent verbal and written communication skills. Ability to work effectively and collaboratively as part of a multidisciplinary team. Strong organizational and time-management skills, with the ability to prioritize responsibilities and manage multiple clients effectively. Ability to maintain confidentiality and demonstrate professionalism in all interactions. Valid driver's license and a clean driving record required.
