Job Description

We are seeking a dedicated Care Coordinator to join our team. The Centralized Care Coordinator is a pivotal role dedicated to enhancing the well-being of individuals and families by facilitating seamless access to essential community resources and services. Through a patient-centered approach, the coordinator collaborates closely with providers, clinicians, and case managers to develop and implement comprehensive care plans. By fostering strong community partnerships and advocating for the needs of individuals, the Centralized Care Coordinator plays a crucial role in promoting, maintaining, and improving overall health and mental well-being. Key responsibilities include conducting thorough assessments, developing tailored treatment plans, providing ongoing support, and empowering individuals to make informed decisions about their healthcare.
Job duties to include:
Conducts assessments, which identify customer problems, needs, and strengths. Conducts treatment planning that is consistent with identified customer needs and follows principles of person-centered planning and prepares a detailed Individualized Plan of Service for each family served. Ensure that person centered plans are completed in a timely manner upon entry to enhanced services and updated at least annually thereafter for primary caseload customers. Completes all annual workflow paperwork including financials, consent to share, consent for treatment, consent for telehealth, no show policy, social determinants of health, level of care tool, and biopsychosocial assessments for all clients and any additional assessments required for specific programming. Completes all assessments, addendums, and transition plans for clients who are transitioning level of care. Review progress, or lack thereof, toward treatment goals. Completes Adverse Benefit Determinations when recommending discharge. Completes 1519(i) SPA benefit form and assist with entering forms into the WSA as necessary. Ensure timely completion of reviews for primary caseload customers and make modifications to treatments plans as needed. Follows principles of self-determination. Coordinates with supervisor to transfer to another program as indicated by consumers needs and or refers to another agency or provider. Coordinates services with other agencies, providers, and significant people (e.g., medical professionals, law enforcement, courts, landlord, AFC provider, family members, guardians, representative payee, day program providers). Plans with the customer for discharge from services as appropriate based on customer needs and progress toward stated goals. Provides advocacy to encourage the customer's inclusion in community life and nondiscriminatory practices with respect to developmental disabilities. Assess, mental status and psychosocial functioning of customers.
