Job Description

Housing Case Manager Have Mercy Position:
Housing Case Manager Position is 32 to 40 hours per week
Description:
The Housing Case Manager's primary role is to: provide assistance in obtaining permanent housing to homeless households by identifying and eliminating housing barriers, establishing a housing plan and assisting clients to self-sufficiency. This position reports to the Program Coordinator and works closely with the Emergency Relief Case Manager.
Essential Functions:
Provide clients with current housing and employment information as needed Provide housing applications and any needed assistance in filling them out Eliminate barriers to permanent housing or find adequate solutions by coordinating with other sources and utilizing strength-based case management Obtain the appropriate documentation necessary for programs Develop and maintain good relationships with outside agencies and landlords. Monitor ongoing documentation including progress notes. Complete statistics and narratives that contribute to monthly, annual and other reports. Work with Emergency Relief Case Manager to obtain permanent housing for clients as rapidly as possible. Other duties as assigned
Qualifications:
Experience in human services field preferred, but equivalent experience in case management is acceptable. Knowledge of adolescent and adult women who are homeless, survivors of domestic violence, sexual assault, at risk of involvement in prostitution, drug use and trafficking, or another street activity is beneficial. Experience dealing with mental illness is very beneficial. Ability to demonstrate compassion, sensitivity, and to respect the privacy and special needs of our clients. Excellent skills in client intervention, assessment and documentation. Familiarity with the Montcalm and Ionia communities and available resources, and experience working with culturally diverse populations is a plus. Physical Demands /
Work Environment:
Occasional local driving and travel required to meet clients, attend meetings, training sessions or conferences Moderate lifting of supplies, groceries and donations required on a regular basis A smiling face and good attitude are required.
Start Salary Range :
The starting pay range is $16.00 - $18.00 per hour and is dependent upon education and experience.
