We help people across Minnesota choose and use Community First Services and Supports (CFSS), including model education, Service Delivery Plans, and ongoing consultation support. About the role We are hiring a Lao-speaking CFSS Consultant to work directly with CFSS participants and their representatives. You will educate peopleLocation requirement (required) Must currently live in the state of Minnesota Must be able to serve Minnesota participants (in person and/or remote as assigned) Office base: Golden Valley, MN 55422 Language requirement (required for this posting) Fluency in Lao (spoken) for participant communication Ability to communicate in English for documentation and DHS systems Preference for candidates who can also write in Lao when needed for participant materials Responsibilities Educate participants and representatives on CFSS so they can make informed decisions Help write and review Service Delivery Plans; confirm plans are complete and only include covered services Document consultation sessions using DHS-6893S or an equivalent form Submit required documentation through DHS systems as directed Provide ongoing consultation within DHS rules and Northstar policy Complete all DHS-mandated training for the role Keep credentials current and follow conflict-of-interest requirements under Minn. Stat. §256B.85 Provide consultation in English and Lao when matched to caseload Required qualifications Currently lives in Minnesota Fluency in Lao Age 18 or older Must pass a Minnesota Statutes Chapter 245C background study (NetStudy 2.0) before providing consultation services

• One year of full-time experience providing direct services to people with disabilities or people older than age 65; OR
• Experience coordinating or directing services for people with disabilities or people older than age 65, including self-directed services; OR
• Experience coordinating your own services
• Ability to communicate in English for documentation and required systems Preferred qualifications Experience with self-direction (CDCS support planning, support broker, case management/care coordination, or consumer education through an FMS or PCA agency) Strong documentation skills and comfort with in-person and remote statewide work within Minnesota Valid driver's license and reliable transportation if field visits are assigned How to apply Email info@cfssnorthstar.

This is a consultation staff role. It is not a participant CFSS support-worker job. Support workers are hired through agencies or FMS providers, not through Northstar consultation staffing.
