Job Description
Overview We are seeking an individual with active Social Work Licensure in the State of Minnesota. This is a PT position (3 days per week from 2:00 pm-6:00 pm). Candidate will travel from the office to the homes of clients in Lakeville and surrounding communities (mileage reimbursement). Candidate will provide supervision and evaluation of client/participant's services. Candidate will provide training, supervision, and competency evaluation of
PCA/CFSS
workers and maintain required documentation. Candidate will communicate with client/participant's case managers and CFSS consultants.REQUIRED
