Job Description

Job Purpose The JOP Case Manager will provide and coordinate services that are an integral part of helping
LIFECORE
Health Group's consumers access needed legal, medical, social, educational, and other services in order to attain their highest level of independent functioning. Additionally, the JOP Case Manger will serve as the liaison between the consumer, parent, and youth courts. Primary Duties and Responsibilities
  • Provide conscientious mental health care services to children with serious/severe emotional disturbance
  • Participate in all training required by the Department of Mental Health and
LIFECORE
Health Group so as to maintain current credentials
  • Adhere to quality assurance guidelines
  • Provide all required documentation pertinent to provision of services
  • Facilitate groups 3 days a week for one hour at JDC
  • Attends all transition meetings at the JDC to gain referrals and track progression/regression of clients served
  • Communicate with youth court officials (youth court counselors, judges, etc) weekly to assist with primary needs of detainees and/or potential consumers
  • Attends all Map Team meetings
  • Other duties as assigned Qualifications
  • Bachelor's degree in Psychology or Social Work
  • Valid Driver's License
  • Valid Car Insurance
  • Reliable Transportation Working Conditions
  • Work location includes JDC, Youth Court, Schools and Homes of consumers
