Addus HomeCare, Inc. seeks a Home Care Service Coordinator/Case Manager to coordinate in-home services for seniors and vulnerable adults. Responsibilities Assess client needs and eligibility for home and community-based services. Develop, implement, and regularly update individualized care plans. Coordinate and schedule caregivers to ensure appropriate coverage and service delivery. Monitor service quality, client satisfaction, and compliance with regulatory standards. Document assessments, care plans, and visits accurately in electronic health records. Collaborate with nurses, social workers, families, and community agencies. Educate clients and families on services, safety, and available community resources. Respond to changes in client condition and adjust services accordingly. Support adherence to Medicaid, Medicare, and payer requirements. Participate in team meetings and contribute to quality improvement initiatives.
