Job Description

Position Summary The Case Manager supports case management services by helping coordinate care, resources, and services for clients. This role works closely with Case Manager/Supervisor to ensure clients receive timely, organized, and compassionate support while maintaining accurate documentation and communication. Key Responsibilities Case Managers with client intake, assessments, and ongoing case coordination Help develop and track service plans based on client needs and goals Coordinate appointments, referrals, and follow-ups with foster parents and DHHS team Maintain accurate, up-to-date client records and documentation Monitor client progress and report changes or concerns to the Lead Case Manager Communicate with clients, foster families, and DHHS in a professional and supportive manner Ensure compliance with agency policies, confidentiality standards, and regulatory requirements Provide administrative support, including data entry, filing, and scheduling Qualifications High school diploma or equivalent required; associate's or bachelor's degree in social work, human services, or a related field preferred Previous experience in case management, social services, healthcare, or a related field preferred Strong organizational and time-management skills Excellent communication and interpersonal abilities Ability to work independently and as part of a team Proficiency with basic computer systems and documentation tools Compassionate, detail-oriented, and client-focused approach Understanding of mental health Sucessfully complete and pass a background check Work Environment May involve office, community, or field-based work depending on program needs Requires interaction with diverse populations and service providers
