Adult Case Manager Pay:
$19.00 to $25.00 per hour, depending on experience

Location:
New Hampshire Healthcare Recruiters International is seeking an Adult Case Manager for a respected behavioral health organization in New Hampshire. This position provides community based support and care coordination to adults experiencing serious mental illness, co occurring disorders, and complex psychosocial needs. The Case Manager helps clients access behavioral health care, medical services, housing, financial assistance, employment resources, and other community supports. Responsibilities Coordinate services across healthcare providers and community agencies Help clients develop goals related to stability, independence, and recovery Connect clients with housing, financial, medical, vocational, and educational resources Provide support in office, community, residential, hospital, and home settings Collaborate with an interdisciplinary treatment team Complete timely and accurate documentation Respond appropriately to behavioral health concerns and client needs Maintain professional boundaries and confidentiality Qualifications Bachelor's degree in social work, psychology, counseling, rehabilitation, human services, or a related field OR Associate degree in a human services field with relevant behavioral health experience Experience supporting adults with serious mental illness or complex psychosocial needs preferred Strong communication, organization, and interpersonal skills Valid driver's license, automobile insurance, reliable transportation, and an acceptable driving record Ability to travel throughout the service area Experience with electronic medical records preferred Benefits and Incentives Health and dental insurance 15 vacation days per calendar year Sick time and paid holidays 403(b) retirement plan with employer contributions Employer paid life insurance, accidental death and dismemberment insurance, and long term disability insurance Employee Assistance Program Student loan forgiveness opportunities Relocation assistance of up to $4,000 Master's degree education incentive providing up to $6,000 per year for five years toward tuition costs Healthcare Recruiters International is managing this confidential search.
