Case Manager - AOTS (G)

Job Description

Position Summary Assisted Outpatient Treatment Services (AOTS) facilitates the Involuntary Outpatient Commitment Law in Somerset County. Under the supervision of the Clinical Supervisor, the Case Manager provides community-based support to people who are struggling to engage in treatment. Must be available to work Monday-Friday, 9:00 am-5:00 pm. Limited on-call availability is required. Essential Functions Responsible for the conduction and completion of initial intake assessments, individual case management sessions, and/or family meetings. Will complete clinically sound documentation in accordance with center policies, state regulations, and Joint Commission Standards. Maintains awareness of the practical and clinical status of all clients in the department, so that individual needs are addressed, using sound judgement in crisis and making referrals to other providers when appropriate. Provides coverage for other case managers within the department when needed. Engages clients and families through outreach strategies utilizing skills that build rapport, boundaries, and ensure safety. Build rapport with community providers to ensure strong communication between teams. Notifies Clinical Supervisor of any issues and updates team on any new initiatives they become aware of. Engages in trainings and activities, including but not limited to in-house training and statewide initiatives, for professional growth and quality service provision to clients. Transports clients to appointments within the community as related to the state services plan. Prepares Court Treatment Plan and Court reports for all clients prior to their individual court dates. Attends al monthly IOC court hearing unless otherwise scheduled off and encourages all clients to attend court hearings. Maintain monthly data of services provided to clients and provides them to Clinical Supervisor no later than the 3 rd of each month. Assists in the compiling and completion of monthly, quarterly and/or annual reports when requested by Clinical Supervisor Perform other duties as assigned by management. Qualifications Essential Master's Degree in Social Work, Psychology, Counseling, or related field from an accredited college or university or Bachelor's Degree in social work, Psychology, Counseling or related field from an accredited college or university with a minimum of three years of experience in working with individuals with severe and persistent mental illness on an intensive mental health case management team, or in an Acute Care setting (Partial Hospital Program, Acute Care Hospital, Psychiatric Screening Center.) Must possess a valid NJ driver's license in good standing. Has experience in working across cultures. Skill in the operation of office equipment, including but not limited to computers, printers, copiers, calculators, and other job specific equipment. Ability to work collaboratively and build relationships with colleagues, coworkers, vendors, contractors, and the public. Ability to accurately prepare and maintain files, records, reports, and logs. Skill in using computer applications including electronic health record, spreadsheets, databases, and word processing software. Ability to identify and maintain confidential information. Ability to comprehend, express and exchange information in English over the phone, in person, in writing or through other communication or electronic devices. Preferred Bilingual ability (English/Spanish) Familiarity with symptoms and evidence-based treatments of major psychiatric diagnosis Familiarity with the psychiatric medication prescribed to the program's target population Knowledge of Wellness and Recovery Principles, Person-Centered Care, Trauma-Informed Care, and Integrated Healthcare. •Candidates with lived experience in mental health and/or substance use disorders and in recovery are encouraged to apply.

$30.05 - $33.65/hour
