Job Description

We are currently seeking an organized and devoted Service Coordinator to join our team!
JOB SPECIFICATIONS
Responsibilities:
Support residents and families with day-to-day needs, concerns, and resources. Welcome new residents and assist with their transition into the community. Coordinate services, referrals, assessments, and community resources based on residents' changing needs. Work closely with the interdisciplinary team, outside agencies, and service providers. Advocate for residents and provide support with sensitive care-related matters, including Advance Directives and POLST when appropriate. Maintain accurate, confidential resident records in accordance with HIPAA. Assist with resident programs, including fall prevention, meals, parking, and other community initiatives. Prepare the monthly resident calendar and newsletter. Coordinate educational, health, wellness, and social programs, including at least three original programs each month. Attend monthly staff meetings and required training. Perform other duties as needed.
Qualifications:
Strong communication, customer service, organization, and problem-solving skills. Ability to work independently and as part of an interdisciplinary team. Knowledge of community resources and government programs. Comfortable working with residents and families in sensitive or stressful situations. Computer proficiency, including Word, Excel, and Outlook. Understanding of confidentiality and HIPAA requirements. Spanish speaking required.
Job Type:
Full-time Pay:
$25.00 - $27.00 per hour
