Job Description

The Involuntary Outpatient Commitment (IOC) program provides community-based coordination and support for individuals who do not meet the criteria for inpatient commitment, but struggle with treatment adherence and require structured care to prevent decline. Serving Monmouth and Middlesex Counties, the Lead Case Manager oversees overall departmental care coordination and staff supervision while holding on-call responsibilities. Key operational duties include managing court system coordination, leading planning for admission, treatment, discharge, and aftercare, and representing the department at community and state meetings. To ensure comprehensive system integration, the position maintains active contact and positive working relationships with referring agencies, local and state hospitals, screening units, treatment providers, and broader community resources. In addition to administrative leadership, the Lead Case Manager provides direct monitoring, screening, and treatment adherence support within a multi-disciplinary framework. The role requires the clinical knowledge and skills necessary to deliver age-appropriate care, understand growth and development principles, and interpret consumer data to address individual needs through agency-approved, evidence-based practices. The Lead Case Manager is responsible for executing SCM, CPR, and First Aid techniques as needed, maintaining active participation in all required annual and job-specific trainings, and fostering supportive connections across each consumer's care network. Hours Position is full time, Monday - Friday. Education/Experience:
