Job Description

Housing Support Services - Case Manager (Per-Diem) Help individuals at risk of or experiencing homelessness achieve housing stability and self-sufficiency. As a Housing Support Specialist, you'll provide client-centered case management across Atlantic and Cape May Counties, connecting people to safe housing and critical support services.
Key Responsibilities:
Conduct outreach in the community to engage and enroll individuals in services Perform assessments and develop individualized service plans Assist clients in securing and maintaining stable housing Provide relocation support, home safety planning, and follow-up care Connect clients with essential resources such as benefits, healthcare, and housing programs Maintain accurate and timely documentation in the Electronic Health Record (EHR) Track outcomes and ensure compliance with Medicaid and agency standards
Position Requirements:
Bachelor's degree in Social Work, Human Services, or a related field Experience in housing support, case management, or working with individuals experiencing homelessness preferred Familiarity with Medicaid documentation and community housing programs Valid NJ Driver's License required At JFS, you'll be part of a passionate, diverse team that's been making a real difference in the community for nearly 100 years.
