Job Description

Per Diem Case Manager - Community Support Services (CSS) Join the JFS team and help individuals with serious mental illness achieve stability, wellness, and autonomy. As a CSS Case Manager, you'll coordinate and deliver care that is safe, timely, effective, equitable, and client-centered, supporting clients as they transition from state psychiatric hospitals to independent community living.
Key Responsibilities:
Assist consumers with transitioning into the community from state psychiatric hospitals Provide medication adherence support for psychiatric and medical treatment Deliver skills training in daily living areas such as household maintenance, finances, meal preparation, nutrition, and interpersonal relationships Promote healthy choices and positive coping strategies Use motivational interviewing techniques to engage clients in their goals Encourage community integration through opportunities and support Document progress notes daily in the electronic health record (EHR) Collaborate with team members and external providers to ensure coordinated care Use electronic devices and tools with ease to support service delivery
Position Requirements:
Bachelor's degree in Social Work, Psychology, Human Services, or a related field Experience in mental health, social services, or related fields preferred Valid NJ Driver's License required Flexibility to work early morning hours (6 AM - 8 AM), evening hours (6 PM - 9 PM), and weekends as needed.
