Job Description
We are seeking a dedicated and detail-oriented Case Manager/Counselor to join our team. This vital role involves managing client care, providing counseling, faciliting groups and ensuring access to necessary resources. Duties Develop and implement individualized care plans for clients Provide counseling and facilitate therapeutic groups. Facilitate access to community resources and services Collaborate with multidisciplinary teams to enhance client outcomes Document client progress and maintain accurate records. Requirements Bachelor's degree in Social Work, Psychology, or related Human Service field. Experience in mental health and social work environments is advantageous.
